This is a directory of health care providers which are participating in one or more CFMC networks.  Participating providers have agreed by contract to reduced rates for the payer and the patient.

Also, be aware that not all health plans contain all the providers listed in this directory.  Some plans use only a part of the network.  Some physicians practicing at participating hospitals may not be in the network.  The participating status of physicians and hospitals both should be verified.

CFMC is a PPO network.   Therefore, we do not determine eligibility, benefits, or covered services.  We are not a health plan or an insurance company, and we do not pay claims.  For questions about these matters, contact the Plan Administration office directly at the number listed on your identification card.
